Private Referral

Our clinic welcomes referrals for private patients seeking specialised pedorthic services.

Private Referral Process :

  • 1. Self-Funding Patients: A referral is not required for self-funding individuals but we can streamline the consultation process. Your health professionals can fill out the Online Patient Referral Form. Once we receive the referral, we will contact you or you can Contact Us.
  • 2. Health Fund Claims: If you plan to claim with your private health fund for custom-made orthopedic footwear and foot orthotic devices, a referral may be required. Contact your health fund to confirm their specific claim requirements..

To assist with your referral or claim, please bring to your appointment:

  • Details of your private health fund.
  • Any relevant medical documentation or previous assessments.

 

EnableNSW

The EnableNSW Aids and Equipment Program provides essential aids and services to NSW residents with lifelong disabilities. As a recognised supplier of Pedorthic Services, we offer various footwear and lower limb orthosis, modifications and repairs.

Enable Application Process:

To apply for assistance through EnableNSW, Please discuss this with your Allied Health Professional. Contact Us to schedule a consultation to assess your needs or request your allied health professional to fill out the Online Patient Referral Form.

Documentation required for EnableNSW Aids and Equipment program application:

  • 1. Consumer Application Form: This can be completed on your visit to our clinic or filled out as an Online Enable NSW Application Form
  • 2. Footwear & Orthotics Request Form: Footwear & Orthotics Request Form. filled out by eligible prescriber.
  • 3. Quotation: Quote for recommended footwear device from us after the assessment for the appropriate footwear.

National Disability Insurance Scheme (NDIS)

As an NDIS registered provider, we offer custom-made orthopedic footwear, medical grade footwear and orthotic devices to agency-managed, plan-managed and self-managed NDIS participants under the Assistive Technology Program.

NDIS Process :

  • 1. Referral: We work within an Allied Health multidisciplinary team. A referral detailing the participant’s health history and assistive technology assessment is beneficial. Required Information to facilitate the referral process:
    • NDIS number, plan start/end dates and contact details.
    • Details of your plan nominee, support coordinator, and plan manager.
    • Copies of the “About Me” and “Goals” sections of your plan.
    • Relevant reports and scans.
    • Referral from a healthcare professional, if applicable.
  • 2. Consultation: Schedule a consultation with us to assess your needs and sign the NDIS service agreement and the Consent Form.
  • 3. Quotes and Approvals: We will provide a quote to the NDIS Coordinator or lodgement centre and await written approval before proceeding with the recommended supply. Discuss recommendations and quotes with your coordinator to align with your NDIS plan.

Aged-care

Our pedorthic clinic offers a specialised footwear service for patients under the Myagedcare Program. We ensure that patients receive professionally fitted medical-grade footwear tailored to their personal needs, minimising the risk of falls and preventing serious complications from medical conditions. A variety of styles are available, including smart and casual dress shoes, walking shoes, summer sandals and supportive slippers. All footwear is lightweight, antibacterial, washable and available in multiple colours, widths, depths and fastening mechanisms.

Process:

  • 1. Consultation: Contact Us directly or request your allied health professional/carer to fill out the Online Patient Referral Form. We will provide a detailed consultation report.
  • 2. Quotes and Approvals: We will provide a quote to your case manager/carer, along with a consultation report for funding availability/approval. We will proceed with the recommended supply once we receive written approval or a purchase order.

Worker's Compensation

Our clinic accepts referrals for individuals covered by workers’ compensation insurance. Your case manager or health professional can fill out the Online Patient Referral Form or Contact Us.

Worker’s compensation insurance Process :

  • 1. Pre-Approval: Secure pre-approval for an initial consultation and assessment from your insurance provider. 
    Required Information to facilitate the referral process:
    • Details of your workers’ compensation insurance company.
    • Case manager's contact information and case number.
    • Any previous medical reports, scans, and healthcare referrals, if applicable.
  • 2. Consultation: Schedule a consultation with us to assess your needs.
  • 3. Quotes and Approvals: We will provide a quote to your case manager along with a consultation report for approval. We will proceed with the recommended supply once we receive a written approval or Purchase order from the worker’s compensation insurance company.

Department of Veterans' Affairs (DVA)

We are a registered provider of the DVA footwear program, offering a Medical-Grade Footwear (MGF) treatment strategy program, which provides eligible clients with reduced-cost or no-cost DVA-approved footwear. We can also provide custom-made footwear and custom-made orthoses upon request from eligible prescribers and approval from DVA.

DVA card holders Process :

  • 1. Appointment with A Podiatrist: Schedule an appointment with your podiatrist to assess your need for medical-grade footwear.
  • 2. Consultation: Schedule a consultation with us to assess your needs - bring along the prescription form from your podiatrist or have them sent to us along with the Online Patient Referral Form. Print the Medical-Grade Footwear Prescription Form. We will contact you for the selection/fitting of the footwear or you can Contact Us
  • 3. Fitting and Claim: Once we receive approval from DVA for the prescription, schedule a fitting appointment or have the shoes sent to your podiatrist to get them fitted.
