We will be pleased to provide you with an assessment, our recommendations, and a quote if you have been assessed for Medical Grade Footwear and it is in your plan under “assistive technology.” Please follow these basic steps:

APPOINTMENTS
Appointments can be made at our clinics – The Ashley Centre, Unit-19, 1A Ashley Lane, Westmead, NSW 2145 or Unit 8, 15 Carrington Road, Castle Hill, NSW 2154, call 1300 246 328
Please note, before we can schedule appointments, all new NDIS participants must complete the online referral form.
STEP 02
What to bring to
your appointment?
- Your Plan ID dates and number
- Your Participant number “43 ……..”
- Your contact details
- A referral from your Doctor, Podiatrist, Physiotherapist, OT, etc
- The name and contact details of your Coordinator
- A copy of your plan if you’d like?
- A pair of socks
- The shoes that you will be leaving for modifications
- The shoes that you will be leaving to have custom moulded orthoses fitted
- The shoes that you will be leaving for repairs
Assessment Fee
There may be an assessment fee for our recommendations and the quote we provide. We will discuss these costs with you.
We will ask you
If you have been assessed for footwear and if you have “assistive technology” in your plan tell us some information about yourself, so we can best help you. Please download and sign our consent form, so we can discuss your plan with your coordinator.

Please fill out and sign our NDIS Service Agreement to proceed.
Agreement form
Following your assessment appointment, we will speak with your Coordinator to discuss how our recommendations and quote fit in with your plan and how it will help to achieve your desired outcomes. We will provide you and the NDIS with a quote. We will wait for the written approval before we can proceed.

Approvals
New Footwear and orthoses
Once we have received written approval from the NDIS, we will start making your footwear. We may need you to come back for measurements or to take plaster moulds/casts and your footwear will take approx. 4-6 weeks to complete.
Assessments/Quotes/Modifications/Repairs
We will contact your Coordinator to discuss whether you have this in your plan and which line item number we use to claim for these services. You will need to leave your footwear with us for modifications and repairs.
Footwear/Orthoses Supply
We will phone you to organise your fitting and collection appointment. This is where we fit you and make sure that your footwear/orthoses are fitting correctly and we discuss the recommended guidelines for “wearing in new shoes and orthoses”.
Checkups
It is a supply requirement that you come back for a check-up within 4 weeks following this “wearing in” period to make sure everything is working right for you.
